3. Proof of viral persistence in serum collected before death; however， collection within two weeks prior to death is required to exclude persistent virus in acute cases which do not present with encephalopathy
4. A presumptive diagnosis based upon epidemiologic evidence without laboratory confirmation， if supported by an expert rabies panel Note： This list does not include special tests such as immunohistochemistry and polymerase chain reaction which can be useful in certain situations but whose negative results will often fail to satisfy the case definition

潜伏期 ：通常为1~3个月，最长可达一年，甚至几十年也未见发作的病例也有报道，一般很少超过一年．所以，如果当年被狗咬伤但未注射疫苗而多年后发作狂犬病的话，则不是当年咬的，而可能是以前咬伤的，因为狂犬病的潜伏能力很强！当然这是极少数的情况了．一般来说，在半年以内者约占98%．但也有研究表明，狂犬病的潜伏期可能更长一些，而且，动物咬伤患者年龄越大，潜伏期越长．有学者报告，1例67岁被家犬咬伤患者长达25年仍有致癫发作，且其家族三代内均有因狂犬病致死者． 前驱期或兴奋期：患者在发病初期，常先有发热、头痛、烦躁不安、厌倦、倦怠等症状，接着进入兴奋期，临床表现为极度恐水、怕风、咽颊粘膜感觉异常疼痛等特征性症状与体征．由于喉头痉挛和水中毒的关系，患者饮水时发生吞咽困难，甚至不能下咽，故称为“恐水症”；因患者不敢喝水，多发生脱水，因此病人常因极度口渴而饮足量水后，又因怕水而多次呕吐，甚至吐出胆汁和肠液，进而出现严重脱水，体重减轻，呈脱水状；患者还可伴有发热、被咬处疼痛麻木及全身不适等症象，此期的精神症状以恐惧焦虑为主，对周围人的安抚和解释不能理解而形成谵妄，有时还表现出恐水、怕风和咽喉痉挛交替出现的特征．临床上，多数患者于病程第6日进入昏迷期，可出现意识错乱，定向力障碍，抽搐和全身强直等典型症状．
